After Care Program
After care program
At Brightpath Care Services, we are dedicated to supporting young people who are transitioning or have transitioned out of the Out-of-Home Care (OOHC) system. Our mission is to empower these individuals as they navigate the journey to independence, providing essential aftercare services tailored to their unique needs.

We help young people find stable accommodation and set up a home, ensuring they have a safe and supportive environment. Our team connects them with education, training, and employment providers, opening doors to new opportunities for personal and professional growth.

Understanding the challenges of independent living, we offer programs to build crucial life skills, such as budgeting and financial management. For young parents, we provide pregnancy support and parenting skills development, helping them thrive in their new roles.

Mental and physical well-being is at the heart of what we do. We facilitate access to counselling and other health services, ensuring our youth receive the comprehensive care they deserve.

At Brightpath, we believe in the power of advocacy. We empower young people to take control of their futures by giving them access to their personal out-of-home care files and a voice in their journey.

Together, we create a nurturing environment where young individuals can build resilience, gain independence, and work towards a brighter future.
